What Is UCL Reconstruction
UCL reconstruction (commonly known as Tommy John surgery) is a surgical procedure that repairs a torn ulnar collateral ligament in the elbow. This ligament is essential for throwing motions and is commonly injured in overhead athletes, particularly baseball pitchers, quarterbacks, and javelin throwers.
During the procedure, Dr. Feghhi replaces the damaged UCL with a graft tendon—typically from your own body (autograft) or from a donor (allograft). This graft is anchored in place, creating a new, stable ligament that allows athletes to return to competitive throwing.

The Procedure
Dr. Feghhi performs UCL reconstruction as an outpatient procedure, using advanced techniques to optimize recovery and outcomes.
You'll receive general anesthesia for comfort. Dr. Feghhi makes an incision on the inside of your elbow and identifies the damaged UCL. The torn ligament is carefully removed and prepared for reconstruction.
A graft tendon is prepared—commonly taken from your own forearm (palmaris longus), hamstring, or from a donor allograft. Dr. Feghhi will discuss the best option for your specific case.
Dr. Feghhi creates bone tunnels in the humerus and ulna bones. The graft is threaded through these tunnels and anchored securely with screws or buttons, creating a new, stable UCL.
The incision is closed with stitches, and your arm is placed in a splint for protection. You'll begin a structured rehabilitation program to restore range of motion and eventually return to throwing.

UCL reconstruction recovery is a gradual process. Most athletes return to competitive throwing within 12-18 months.
